Audit log

Every significant change per PRD §15: entity, entity_id, action, old_value, new_value, changed_by, timestamp, ip_address. Read-only — entries are written by the system, never edited.

TASK mapping. TASK-050 writes one row per significant change across all modules; TASK-051 is the finance subset (invoices, payments, CN/DN, kasbon — every row shows old → new values); TASK-052 is the document subset (upload, version, generate, download access); TASK-053 is user activity history (logins, permission changes). Rows are immutable and queryable; export honors the current filter scope. Users see only their own activity — that subset renders inside Administration for Super Admin here, and as "my activity" for other roles in a later pass.
